Adhoc Nurse Recruitment :

Applications are invited for the following AD HOC posts for HOMI BHABHA CANCER HOSPITAL & RESEARCH CENTRE, Vizag, Visakhapatnam The appointment is initially for a period of one year only and extendable, if required.

Post : Adhoc Nurse
Norms : General Nursing & Midwifery plus Diploma in Oncology Nursing with 1 year experience OR Basic or Post Basic B.Sc.(Nursing) with 1 year oncology experience. Candidates should be eligible to register with Andhra Pradesh Nursing Council.Hepatitis Vaccination should be completed.
Consolidated remuneration Per Month : 18,000

Post : Adhoc Assistant Nursing Superintendent
Norms : M.Sc. (Nursing) with 3 years’ experience OR B.Sc. (Nursing) with 10 years’ experience as Nursing Administrator out of which minimum 2 years experience in Clinical Field. Preference will be given to candidates having administrative experience.
Consolidated remuneration Per Month : 45,000-50,000

[ A ] Online Application :
– The application consists of five screens.
– It is mandatory to fill all the fields that are marked with an asterisk *.
– DO NOT FORGET to print out of the Online Application Form And submit by post To: H.R.D.Officer, Tata Memorial Hospital, Dr.E.BORGES ROAD, Parel, Mumbai–400012.

Screen 1 – Please fill Personal information:
** SAVE – to save the filled information.
** NEXT – to go to next screen.
** You should note down your Application ID and password or check your e-mail in case if you forget Application ID and password.Using this Application ID and password, you can access your Application Form by using UPDATE APPLICATION TAB.

Screen 2 – To fill Education information and Current Employment details:
** Education Qualifications–fill the information in provided fields, and click on “SAVE” for each qualification.In case if you want to MODIFY ,click on the srno of the post ,make changes in respective text boxes and then click on MODIFY button.
** Current Employment details
** Click BACK button – to correct mistakes.
** Click SAVE Button – to save the filled information.Click on SAVE Button everytime you make changes and move to NEXT page
** Click NEXT Button- to go to next screen.

Screen 3 – To fill Professional Experience And Gap in Experience information:
IF THERE IS GAP BETWEEN TWO EXPERIENCE DETAILS THEN CLICK YES AND MENTION THE REASON FOR THE SAME IN THE BOX PROVIDED.

** Professional Experience– fill the information in pre-defined fields, and click on “Add” for each qualification.In case if you want to MODIFY ,click on the SRNO of the post ,make changes in respective text boxes and then click on MODIFY button.

** Click SAVE to save the filled information .Click on SAVE Button everytime you make changes and then move to NEXT page.
** Click NEXT Button- to go to next screen.

Screen 4 – To fill Other Information And Referee Details:
** Click BACK button to go back to previous screen.
** Click SAVE button to save the filled information.Click on SAVE Button everytime you make changes and then move to NEXT page
** NEXT – to View print out of application form.
** In case if you have not taken print, click on the submit and print option on above tab,provide your userid and password and then submit and take PRINT
** You will receive an email confirming submission.After which You will not be allowed any changes in Application

** Unless you click on the SUBMIT button, your application will not get submitted into our database. If you send us a printout of this page, without submission, your application will be considered invalid and therefore rejected.

** CLICK on SUBMIT AND PRINT OPTION – to take print of the form.

Screen 5 – Payment Screen — Payment online (Credit/debit Card)payment:
** Payment may be made Credit Card.
** Select Gender Information
** Select online payment and fill Payment details and then click Submit button.
** Female Candidates can click on the Female option to go to Final Screen(Remember only Female candidates are Exempted from online payment).

** If Online/Credit card Payment:-select Credit Card option ,then select either of the fee option according to your category(amount as per mentioned in Notification) And click submit form button on same screen.Then on Next Screen PAY NOW page will appear.

Follow the instruction and make online payment.Then take print of Receipt of successful payment and click on bottom link to come to print preview page to make Final Submission and take PRINT.

** In case if you have not taken print, click on the submit and print option on above tab,provide your userid and password and then submit and take PRINT
** You will receive an email confirming submission.After which You will not be allowed any changes in Application

[ B ] Application Submission :
Payment :
** The candidate must submit online receipt of online payment,a recent passport size photograph, attested copies of all certificates regarding Date of Birth, Educational qualification, Experience Certificates, Caste Certificate, Physically Handicapped Certificate etc. along with the copy of online application form . Last date for online application is mentioned in notification.

** Female candidates and Physically Handicap candidates are exempted from paying fees through online payment.
** Only committed applications will be accepted. The candidate will be responsible if the application is not committed online and submitted to H.R.D. Department.

** Print out of Online Application, along with attested copies of certificates & Demand Draft or receipt of online payment(zerox copy) must be submitted before last date otherwise the candidature of candidate will be treated as cancelled.

IMPORTANT :
** Keep a photocopy of the Online Application (with Photograph) and originals of all the attachments, if called for the Interview.
** Do not submit CV or reprint.
** Incomplete forms will be summarily rejected.
** Please fill all fields marked with an asterisk (*) in the application form, attach photocopies of relevant documents and certificates and sign the declaration.
** All photocopies will be verified.
